Our Transitional Age Youth Services (TAY) provide specialized transitional planning for youth who are funded by the Ministry of Children, Community and Social Services to prepare the individual, their family, and their team for the transition from child to adult-funded support and services. TAY Coordinators work to ensure services and supports meet legislative requirements and work in collaboration with the individual’s circle of care to create service opportunities. TAY Coordinators also work to profile youth issues and gaps in service through committee membership, focus groups, and training events.

“As a Transitional Age Youth (TAY) Coordinator, it has been a privilege to work collaboratively with community partners, circle of care teams and transitional aged youth to capture the true essence of person centred planning. Being a TAY Coordinator ensures all voices are being heard and that the most successful avenue for adult planning occurs. Navigating the world of adult supports and services can be daunting or even stressful for youth and their circle of care teams, TAY helps ensure that all appropriate adult services and supports are in place in order for the young adult to be best supported and successful in their adulthood. “
