Adult Case Management is a collaborative, inclusive, and individualized process. We work towards goal setting, planning, advocating, and exploring available community resources to support the achievement of personal outcomes.

Adult Case Management (ACM) services provide ongoing specialized case management support and oversight within Ontario. Supports include regular communication and check-in meetings with service users and service providers to ensure identified goals are met and adjusted to changing support and administrative needs. Financial oversight of individualized funding, as well as submission of service user invoices and expenses, is also provided along with the maintainance of multiple legislative requirements.

Who is eligible?

To be eligible for ACM Services, individuals must:

  • Receive a referral from the Ministry of Children, Community and Social Services (MCCSS)

  • Or be a youth transitioning to adult services with an approved adult budget

How do I access services?

Referrals to ACM can only be made by MCCSS. For more information, please contact our Central Navigation team.

  • When you call Central Navigation, they will answer your questions and if applicable schedule you an intake appointment with a Specialist. The intake will take place over the phone and last approximately 45 minutes. The specialist will ensure they gather the relevant information to match you to the relevant Canopy Services.

Canopy Support Services Spotlight - Meet Jared Armstrong

Adult Case Management Services Supported Person

Jared has been volunteering at Johnston’s Greenhouse in Peterborough for over 21 years. He spends his time filling the pots with soil and planting beautiful baskets for customers as well as Johnston’s own gorgeous displays. Jared has helped create the hanging baskets and planters in downtown Peterborough which he enjoys showing to his staff and peers. Canopy staff have also had many opportunities to purchase his flowers and delicious herbs. In his spare time Jared tends to his indoor and outdoor home gardens where he plants vegetables for homemade salsa and tomato sauce and a collection of Hosta’s that he sells curbside during the summer.
